Share the DONATE LIFE message wherever you go!

The Donate Life license plate gives every Wisconsin driver an opportunity to call attention to the need for more organ, tissue and eye donors. Each license plate purchase benefits Donate Life Wisconsin and supports its work to increase awareness about the importance of becoming a registered donor.

DONATE LIFE license plates can be purchased for any of these vehicles:

  • Automobiles
  • Mobile homes
  • Trucks (4,500, 6,000 or 8,000 pounds gross weight, including dual purpose farm and dual purpose motor home; 12,000 pounds gross weight farm truck.)

License plates must be ordered from the Wisconsin Department of Transportation (DOT). DMV Service Centers do not issue Donate Life license plates.

Each license plate includes the DONATE LIFE symbol, which represents the lives that are saved or improved because of organ, tissue and eye donation.
